Ingenuity First and ThedaCare Online Privacy & Security Policy
Keeping your personal medical information private Ingenuity First, an initiative of ThedaCare, is committed to providing you with quality health care assessments and advice and to maintaining a relationship with you that is built on trust. This trust is built, in part, on our commitment to respect the privacy and confidentiality of your personal medical information. Although Ingenuity First collects medical information about you on behalf of your employer, ThedaCare owns and stores the information collected. Your employer will have access only to summary information that in no way identifies you personally. Only ThedaCare and Ingenuity First employees will have access to your personal medical information, and each employee knows that such information is available to them only as necessary to provide health care services to you or for other authorized, legitimate reasons. ThedaCare has put into place detailed policies and procedures regarding access to all personal medical information by our staff and employees and has carefully outlined the circumstances under which your personal medical information may be released to parties outside the organization. These policies conform to state and federal law and are designed to safeguard your privacy. Ingenuity First staff and employees are trained in the ThedaCare confidentiality policies and the appropriate use of personal medical information. A ThedaCare employee’s violation of confidentiality policies is considered a serious offense. Failure of an employee to protect personal medical information from accidental or unauthorized access is not tolerated. Communication with Ingenuity First and ThedaCare Staff As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including the results of all tests, procedures and therapies. Communications you have with Ingenuity First medical personnel may be made part of your permanent medical record at the discretion of your health care provider. Once it is made part of your record, it will be accessible to current and future ThedaCare staff members who are appropriately involved with your care. For all urgent matters, please contact your ThedaCare Physician by telephone, go to an emergency room, or dial 911. Restrictions From time to time, users may be asked to complete surveys via the thedacare.org and ingenuityfirst.org Web sites. ThedaCare and Ingenuity First may analyze information submitted via their Web sites as part of descriptive (demographic) studies and reports.
